RAMS Level 1

Remote Adventure Medicine

A practical, expedition-focused medical training programme designed exclusively for OGA Level 2 Guides progressing toward the Level 3 Guide qualification. This course bridges the gap between foundational first aid and the realities of managing incidents in remote, high-consequence environments — developing the judgement, structure, and leadership required to operate at Level 3.

Prerequisite
Remote First Aid at Work (or equivalent recognised qualification)

Operating where Emergency Response Services are limited or absent. RAMS Level 1 directly supports the development of these core Level 3 guide capabilities:

Incident Leadership & Decision-Making
Structured approach using DRCABCDE with advanced interventions including pharmacology. Prioritisation under pressure with limited resources and defensible decision-making in remote environments.

Risk Management in Dynamic Environments
Ongoing scene assessment and hazard control. Balancing casualty care with group safety, environmental risk, escalation thresholds, and evacuation triggers.

Clinical Reasoning & Casualty Assessment
Moving beyond basic first aid into structured clinical thinking. Identifying patterns of illness and injury in expedition settings and recognising deterioration early.

Remote Medical Intervention
Practical interventions where evacuation is delayed. Managing trauma and medical conditions with limited equipment, including oxygen, adjuncts, and assisted rehydration strategies.

Pharmacology & Intervention Planning
Expedition medication planning and risk profiling within a remote governance framework, including access to OGA Doctor Prescription Services. Administration routes including IM and IO principles.

Communication & Coordination
Clear communication under stress, liaison with emergency services and remote support systems. Managing group dynamics during active medical incidents.

Professional Accountability
Documentation, reporting, and post-incident reflection. Operating within an expedition clinical governance structure — understanding personal scope, limits, and decision justification.

Scene management and dynamic risk assessment
Primary and secondary survey in remote settings
Airway positioning and basic adjunct use
Oxygen therapy and monitoring
Catastrophic bleed control — tourniquets, haemostatic agents
Immobilisation and casualty packaging for evacuation
Assisted rehydration strategies — oral and supported methods
Recognition and early management of expedition medical conditions
Communication and coordination within remote response systems
Documentation and incident reporting
Expedition medication awareness and drug planning
Intramuscular (IM) delivery and introduction to intraosseous (IO) access principles
Application of essential medications aligned to environment, remoteness, and operational risk
